Bridging the Follow-Through Gap: A Closer Look at Skill Implementation Challenges
Through our industry research survey conducted this year, practitioners identified a number of challenges clients and therapists face when translating therapy into action.
Clients face a range of challenges when trying to translate therapy into meaningful change. One of the most common is goal adherence—clients often start strong but struggle to maintain alignment with their objectives. This is compounded by overwhelm and inertia, where new concepts feel like too much, too fast, leading to stalled momentum. Many also arrive at sessions with unstructured preparation, unsure of what to reflect on or practice. As life gets busy, attendance and prioritisation issues emerge, and therapy slips down the list of daily responsibilities.
Without wraparound support, sessions can feel disconnected and isolated, offering little continuity between meetings. Skill retention difficulties are common; clients forget what they’ve learned or how to use it effectively. When skills are not embedded into daily life, there’s limited behavioural integration, and progress remains theoretical. Without reinforcement, clients may start losing momentum. They also frequently struggle with uncertain clinical progress, unsure if they’re making meaningful improvements, which leads to disengagement. This can cause a perceived value misalignment, where the therapy’s benefit is questioned. Furthermore, between-session disconnect results in isolated conversations without clear links to personal goals. Many experience a lack of therapeutic clarity, uncertain about the strategy or destination of their sessions. As a result, there's often high between-session reliance, where clients depend heavily on therapists to drive change, rather than building autonomy.
Practitioners aren’t immune to the very challenges they help others navigate. Access to continuous professional development (CPD) is often costly or out of reach, leaving clinicians without updated tools or techniques. Many work in isolation, without a collaborative network, which can limit professional growth and support. A common frustration is when clients struggle to apply therapeutic tools between sessions, stalling their progress.
